Our stabilization services provide comprehensive crisis support, from immediate mobile response to community-based stabilization. We partner with hospitals, jails, clinics, residential programs, and CSBs to ensure seamless transitions for individuals stepping down from intensive care.

Community Stabilization (S9482)
At Foundations to Success, LLC, our Community Stabilization service provides short-term, person-centered mental-health support for individuals who are transitioning from a higher level of care — such as hospitals, jails, clinics, or residential treatment programs — and need structured assistance to safely reintegrate into the community. This program serves as a step-down service focused on restoring stability, promoting recovery, and linking individuals to the next appropriate level of care.
- 24/7 crisis intervention, response, and safety planning
- Psychotherapy and coordination for medication management
- Crisis education and prevention training for clients and families
- Restoration of skills and health literacy coaching
- Mental Health Skill-Building and Life Skills Development
- Substance-use evaluations and linkage to ongoing therapy or treatment
- Psychological evaluations and coordination with outpatient providers
- Resource navigation and housing advocacy
- Restoration of insurance benefits and social security navigation assistance
- Coordination with primary care, psychiatric, and community supports

Mobile Crisis Response (H2011)
When someone is in crisis, every second matters. The Foundations to Success, LLC Mobile Crisis Response team provides on-site, real-time support for individuals experiencing a mental-health or behavioral crisis. We partner directly with the 988 Suicide and Crisis Lifeline and Behavioral Health Link (BHL) system to ensure rapid response and coordination across the community.
- 24/7 mobile response through the 988 and Behavioral Health Link (BHL) network
- Crisis assessment, de-escalation, and safety planning on-site
- Short-term counseling and stabilization to prevent hospitalization or law enforcement involvement
- Family support and crisis education to promote understanding and safety
- Linkage to Community Stabilization and other ongoing services
- Coordination with local CSBs, hospitals, schools, and law enforcement

Crisis Stabilization (Clinic-Based)
Crisis Stabilization is provided within our community-based clinic in the Newport News area. It provides assessments and stabilization interventions to individuals experiencing a behavioral health crisis. Registered Nurses on staff check vitals and monitor progress. Mental Health Nurse Practitioners and psychiatrists assess individuals to determine the level of care needed after observation. This service prevents unnecessary hospitalization.
