Overview

Our clinicians come to you. Sessions take place in your home, school, or community and focus on building skills, strengthening family dynamics, and addressing behavioral or emotional challenges in real-life settings.

Family Support Service

Ages: 5-65 years of age

Family support functions as a team of individuals that systematically place various ongoing resources, organizations and providers to individuals and their families. The family support teams focus on Care Coordination, Continuity of Care, and Family/Individual Coaching.

  • 1-3 Family Support Specialists can be designated for 1 individual and their family
  • Care Coordination — From resources to effort, all we do is for a better community
  • Continuity of Care — We work tirelessly to help those in need
  • Family/Individual Coaching — Introduces individuals and families to alternative resources and organizations beneficial to their well-being
  • Provides critical thinking and problem-solving strategies to help navigate a healthy and successful level of support

Independent Living Services

Ages: Under 21

Independent Living Services help youth develop essential life skills for self-sufficiency and successful transition to adulthood.

  • Debt management, conservation & budgeting
  • Hygiene
  • Health & wellness, nutrition
  • Personal responsibility, job readiness
  • Community resources — bill pay assistance, house hunting, public transportation
  • Communication / public speaking
  • Generating and maintaining effective positive relationships
  • Feeling safe and secure in all settings
  • Building confidence in their abilities
  • Casey Life-Skills Assessment is utilized to assist in guiding treatment

Ages: Under 21

These services are intended for individuals under the age of 21 that need support, coaching, motivation, and training in key developmental areas.

Therapeutic Mentoring Services

Intensive In-Home Counseling

Ages: 5-18 (individuals over 18 but under 21 will be reviewed)

These services prevent out-of-home placements (e.g., foster care, hospitalization) and aid reunification. Clients receive 8-10 weekly hours (up to 15 in crises). We use strength-based, solution-focused interventions to improve behaviors, communication, and interactions at home, school, and in the community. Services begin within 24 hours of referral, with 24/7 crisis support.

  • Individualized treatment plans collaboratively designed with the individual, guardian, and collateral agencies
  • Dynamic treatment modalities: individual and family counseling, therapeutic mentoring, parenting skills, behavior management, relapse prevention training
  • Coordination of care across settings (home, school, work, and the community)
  • Concentration on all domains: social, emotional, cognitive, and physical
  • Identify triggers, cues, and symptoms
  • Case management including links to community resources and court advocacy
  • 24-hour crisis intervention & stabilization
  • Addresses: mental health, substance abuse symptoms, parenting skills, school issues, delinquent behaviors, anger management, blended family issues, conflict resolution

Mental Health Support Services

Ages: 18 and older

Foundations to Success, LLC offers mental health skill-building for adults 18+ with serious mental illness affecting independent living. Services include 1-3 hours per session, using goal-oriented, solution-focused, and evidence-based approaches. Clients undergo an initial screening and assessment, then collaborate with staff to create a personalized treatment plan.

  • Individualized and customized treatment plans
  • Practical skills training
  • Safety plan development and implementation
  • Monitoring of mental and physical health
  • Teaching independent living skills: medication management, personal hygiene, nutrition, food preparation, financial planning, employment skills, and coping skills
