Family/Individual coaching helps to introduce the individual and their families to alternative resources and organizations that will be beneficial to their well-being. The family support specialist clarifies and expound on referrals made to the family to ensure they understand the services and resources they are provided, whether they are mandated or optional. The family support specialist also provides critical thinking and problem-solving strategies to help navigate a healthy and successful level of support for individuals and their families.
The CLS is a set of free tools designed to help youth assess and develop essential life skills for achieving long-term goals and leading healthy, productive lives. It focuses on key areas like maintaining healthy relationships, effective work habits, self-care, study skills, financial literacy, community resource use, and daily living activities. By encouraging self-evaluation, it empowers young individuals to take proactive steps toward their futures.
The CLS is paired with a Character Development Curriculum, which strengthens core values through instruction, exercises, strategies, and hands-on training, focusing on six key qualities. Together, these tools provide a comprehensive approach to preparing youth for success.

Qualities that will demonstrate self-awareness, self-management, self-esteem, emotional regulation, anger management, and mental health wellness.
Qualities that help with hygiene, self-discipline, exercising, stress management, health & wellness, nutrition and food preparation
Qualities that challenge individuals to navigate their life with a more open-minded perspective. Individuals will learn how education, occupation, ethnicity, ethics, religion, economics, and cultural influences impact their ecology.
Qualities that will challenge the mindset of individuals to become more critical thinkers and creative problem solvers. Individuals will be able to demonstrate intellectual integrity and apply knowledge to promote a better future.
Qualities that will influence individuals to determine their leadership style, understand the dynamics of teamwork and teambuilding, social awareness, personal and professional relationship skills and communication skills.

Evaluating the patient’s mood, thoughts, perceptions, and behaviors.

Acquiring insights into the patient’s medical history, current psychiatric mental health condition, symptoms of serious mental illnesses, behavioral disorders, chronic health issues, substance use, and other significant factors.

In certain instances, a physical examination may be warranted to exclude any underlying medical issues that could be affecting mental health status.

Depending on the circumstances, the psychiatric mental health nurse practitioner may employ psychological tests to gain a more profound understanding of the patient’s condition.

Following the assessment, the psychiatric mental health nurse practitioner will establish a diagnosis, create a treatment plan and provide recommendations.
Engaging in discussions with the individual undergoing evaluation, as well as with family members, friends, and others who are acquainted with them.
An examination of medical records and relevant documents pertaining to prescription medications, mental health, and overall physical health.
The evaluation may incorporate a drug screening as part of the assessment process.
Psychological assessments and or psychiatric evaluation may also be included in the SA evaluation.
Evaluations of behavior may form a component of the overall assessment.
1. Individualized treatment plans which are customized and collaboratively designed with the individual, guardian, Home Based Counseling worker, and collateral agencies involved.
2. A multitude of dynamic treatment modalities including, but not limited to: individual and family counseling, therapeutic mentoring, parenting skills, behavior management, relapse prevention training, personal care education, conflict resolution, anger management, supervised visitation, and discharge planning.
3. Coordination of care across settings (home, school, work, and the community)
4. Concentration on all domains of functioning to include social, emotional, cognitive, and physical
5. Identify triggers, cues, and symptoms.
6. Case management to include links to community resources and advocacy in court.
7. Communication and participation with schools, state agencies, doctors, and others is utilized to collaborate in order to go above and beyond the standards of treatment
8. 24-hour crisis intervention & stabilization
9. Address areas such as: mental health and substance abuse symptoms, parenting skills training, crisis response, school attendance and other school-related issues, alcohol and drug use issues, delinquent behaviors such as gang violence, anger management, blended family issues, and conflict resolution.
