Community Stabilization

Bridging the Gap Between Crisis and Long-Term Care

At Foundations to Success, LLC, our Community Stabilization (S9482) service provides short-term, person-centered mental-health support for individuals who are transitioning from a higher level of care — such as hospitals, jails, clinics, or residential treatment programs — and need structured assistance to safely reintegrate into the community.

This program serves as a step-down service focused on restoring stability, promoting recovery, and linking individuals to the next appropriate level of care.

Who We Serve:

Individuals discharging from psychiatric hospitals, crisis units, detention centers, or residential programs

Clients referred by probation, social services, or community service boards (CSBs)

Adults and youth who recently experienced crisis and need short-term clinical and case management support to prevent relapse or readmission

Our Community Role:

FTS partners with hospitals, jails, clinics, residential programs, and CSBs to provide seamless transitions for individuals stepping down from intensive care. We help bridge system gaps by ensuring every person has the tools, skills, and supports necessary to remain stable and safe in the community.

Our Goal:

To help individuals stabilize emotionally, rebuild essential life skills, and reconnect with support systems—preventing re-hospitalization, incarceration, or crisis recurrence.

Mobile Crisis Response (H2011)

When someone is in crisis, every second matters. The Foundations to Success, LLC Mobile Crisis Response team provides on-site, real-time support for individuals experiencing a mental-health or behavioral crisis.

We partner directly with the 988 Suicide and Crisis Lifeline and Behavioral Health Link (BHL) system to ensure rapid response and coordination across the community.

Our licensed clinicians and qualified professionals meet individuals wherever they are—home, school, hospital, or community setting—to stabilize, de-escalate, and connect them to ongoing care.

Crisis Stabilization

Crisis Stabilization is provided within our community-based clinic within the Newport News area.  It provides assessments and stabilization interventions to individuals experiencing a behavioral health crisis.  Registered Nurses on staff at Foundations to Success, LLC check vitals as well as monitor the progress of individuals in an acute crisis as they provide a safe environment.  Mental Health Nurse Practitioners and psychiatrists on staff, assess these individuals to determine the level of care needed after observation.  To prevent unnecessary hospitalization, this service allows for the opportunity to thoroughly assess the individual in crisis and provide psychosocial needs and supports.

Rapid Response and Early Intervention for Community Crisis Situations

A crisis team provides rapid, on-site support to de-escalate individuals in crisis. They engage, observe, and offer solutions, along with follow-ups for 3-5 days (including weekends). The team connects individuals to community resources for stabilization and symptom management. A strength-based crisis prevention plan is created with the individual and their supports to reduce recurring crises and promote long-term stability.

Services last 1-3 days.
