23 Hour Crisis Stabilization

23-Hour Crisis Stabilization is provided within our community-based clinic within the Newport News area.  It provides assessments and stabilization interventions to individuals experiencing a behavioral health crisis within a period of up to 23 hours.  Registered Nurses on staff at Foundations to Success, LLC check vitals as well as monitor the progress of individuals in an acute crisis as they provide a safe environment.  Mental Health Nurse Practitioners and psychiatrists on staff, assess these individuals to determine the level of care needed after the 23 hours of observation.  To prevent unnecessary hospitalization, this service allows for the opportunity to thoroughly assess the individual in crisis and provide psychosocial needs and supports.

Rapid Response and Early Intervention for Community Crisis Situations

Mobile Crisis Response

Services last 1-3 days.

Individuals are supported with a crisis team that works fast and immediately to de-escalate individuals within a crisis in various settings.  The objective is to engage, observe, listen, communicate options to correct crisis as well as provide preventatives, with follow ups for the next 3-5 days (including the weekends).

Individuals are provided strong community linkages and referrals to help stabilize unwanted behaviors and help with symptom management.

  1. Suicidal or Homicidal thoughts, ideations and behaviors

    • Includes cutting, self-harm, threats, etc.

  2. Domestic Violence and/or Rape

  3. Trauma of some sort resulting in a change in mood, regulation, behavior, thoughts, physical and mental well-being

  4. Suspension from school/work

  5. Family death(s)

  6. Mental instability

  7. Homelessness

  8. Criminal or judicial 

  9. Children & Adolescents struggling with behavior, emotional, and mental challenges within school, at home or the community.

  10. At-Risk Youth of being placed out of the home and into alternative housing such as Foster Care

Typical types of crisis:

Each individual and their supports will receive a crisis prevention and education plan that is strengthen based and developed along side of the crisis team.  This is used to help crisis cycling behaviors and also avert any further crisis.

Community Stabilization

Individuals within the luxury of their own home can benefit from this program and will be given many skills and resources to help empower their mental and behavioural health.  This type of program is typically seen after Mobile Crisis Response or transitioned from other higher level of care services such as hospitalization.
